Important information concerning a Marriage License

When issuing a marriage license the New York State Department of Health requires the following acceptable forms of I.D. for current names:
Group A: Items A1 and A6 MUST HAVE A PICTURE and be current:
1. Driver’s License
2. Non-Driver’s License
3. Passport (valid)
4. Naturalization papers – only original papers are acceptable.
5. Military I.D.
6. Employees photo I.D.
Group B: Items B1 and B2 must be translated into English if in another language
1. Birth Certificate
Each partner must upload online and present in person one ID from Group A and one ID from Group B.
Only the name that appears on the documentation is acceptable. We cannot “Americanize” a name. The Bride and groom may not use their current name as a middle name. If both names are used they must be separated by a hyphen.
Only certifiable translators are acceptable.

Al emitir una licencia de matrimonio, el Departamento de Salud del Estado de Nueva York requiere las siguientes formas de identificación para los nombres actuales:

Cada aplicante debe añadir en línea y presentar en persona una identificación del Grupo A y una identificación del Grupo B.
Sólo el nombre que aparece en la documentación es aceptable. No podemos "americanizar" un nombre. La novia y el novio no pueden usar su nombre actual como segundo nombre. Si se utilizan ambos nombres, deben estar separados por un guion.
